Rapha Family Wellness Center has CDC Full Recognition to deliver Medicare Diabetes Prevention Program (MDPP). MDPP is a 12 months structured intervention program with the goal of preventing type 2 diabetes in individuals with an indication of type 2 prediabetes. Full Recognition is reserved for programs that have effectively delivered a quality, evidence-based program that meets all of the standards for CDC recognition.

Our program is a high-intensity intervention program that focuses on holistic wellness and lifestyle modification. Participants are assigned Fitness Coach to help them develop optimal physical exercises and routines that take their abilities into consideration. Our fitness center is equipped with great workout equipment and is open 6 days in a week.

A Health Coach will facilitate all the class sessions. These classes are an hour long and designed to educate participants about healthy nutrition and how to make healthy choices. Depending on the session that is being taught a dietician, clinician or culinary/chef staff member might be facilitating the session. There is a strong emphasis in our program that healthy choices start by participants preparing most of their meals.

  • A positive screening for prediabetes based on the CDC Prediabetes Screening Test
  • Must have BMI => 25 kg/m2 (=>23 kg/m2, if Asian American)
  • Must be 18 years or older and not pregnant at the time of enrollment
  • Participants must be considered eligible based on either:
    • A1c of 5.7 to 6.4
    • Plasma glucose measured 2 hours after a 75 gm glucose load of 140 to 199 mg/dl
    • Fasting glucose of 100 to 125 mg/dl
    • Clinically diagnosed gestational diabetes mellitus (GDM) during a previous pregnancy
  • Diabetes stats
